Position Overview

We are seeking compassionate and reliable Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to join our growing home health team. Nurses will provide skilled clinical care, support chronic disease management, promote patient independence, and collaborate with an interdisciplinary team to optimize outcomes.

RNs will conduct comprehensive assessments, complete OASIS as needed, develop nursing care plans, and oversee clinical direction. LPNs will provide direct skilled care under the RN-developed plan of care and communicate patient progress or concerns.

Both roles offer flexible visits, independence, and the support of a responsive and collaborative clinical leadership team.

Location

This position serves patients across multiple communities throughout Massachusetts. Nurses from any region of the state are encouraged to apply. You may set your preferred coverage radius and availability.

Key Responsibilities For Registered Nurses (RNs)
  • Perform initial and ongoing comprehensive patient assessments, including OASIS assessments when required.
  • Develop, implement, and revise individualized nursing care plans in collaboration with physicians and interdisciplinary staff.
  • Provide skilled nursing interventions such as wound care, catheter care, ostomy care, medication administration, injections, IV therapy (as applicable), chronic disease management, and patient education.
  • Monitor and evaluate patient progress, response to treatment, and changes in condition.
  • Communicate regularly with physicians to update care plans, request or clarify orders, and report significant changes (at least every 30 days).
  • Supervise LPNs and Home Health Aides (HHAs) per agency policy, including scheduled supervisory visits.
  • Participate in interdisciplinary case conferences, staff meetings, and ongoing professional development.
  • Complete comprehensive discharge planning and required documentation.
For Licensed Practical Nurses (LPNs)
  • Provide skilled nursing treatments and interventions under the plan of care developed by the supervising RN.
  • Perform wound care, medication administration, catheter care, vital sign monitoring, patient education, and other nursing tasks within LPN scope.
  • Observe, document, and report changes in patient condition to the supervising RN.
  • Reinforce education and instructions provided to patients and families.
  • Collaborate with HHAs and other team members to support safe, coordinated care.
Shared Responsibilities (RNs & LPNs)
  • Deliver safe, skilled, patient-centered home health care.
  • Document all visits, interventions, patient responses, and education according to agency standards (same-day documentation, filed within 48 hours).
  • Assist patients and caregivers in managing health needs and promoting independence.
  • Participate in maintaining continuity and quality of care across the clinical team.
  • Promote patient safety through ongoing assessment, communication, and education.
Admission Support (RN Only)
  • Conduct full patient admission assessments, including medical history, medication reconciliation, home safety evaluation, and functional assessment.
  • Confirm accuracy and completeness of physician orders and follow up for clarifications as needed.
Qualifications Education & Licensure
  • Graduate of an accredited RN or LPN nursing program.
