Job Overview

The Street Level Caseworker provides mobile outreach, relational case management, and community navigation. The primary objective is to walk alongside those experiencing homelessness, identify and overcome barriers, and connect them with resources in the community. Working with Phoenix Family Services and the Street Outreach Program, the caseworker facilitates mobile outreach, heat relief, and community networking events. They maintain case files, track financial assistance, manage resource inventories, and collaborate with agencies to provide referrals.

Qualifications
  • Associate degree in Human Services or related discipline; 2 years of direct case management experience may substitute for degree.
  • Experience working with clients in crisis; skilled in trauma-informed care, motivational interviewing, and de-escalation.
  • Bilingual English/Spanish preferred.
  • Experience with homeless, at‑risk veterans, and families preferred.
  • Strong knowledge of the cycles that affect at‑risk individuals/families and homelessness.
  • Knowledge of community resources, especially low‑income housing, in the service area.
  • Assisted with social service client intake in the field.
  • Proficient in Microsoft Office (Word, Excel, PowerPoint, Publisher), Adobe Acrobat, email, and internet.
  • Excellent interpersonal, telephone, and written communication skills.
  • Ability to work effectively with diverse consumers, staff, and community partners.
  • Professional and courteous conduct consistent with Christian leadership standards.
  • Excellent detail, organization, and file‑management skills.
  • Dependable, mature, initiative‑driven; maintain confidentiality and treat everyone with dignity and respect.
  • Self‑motivated with ability to manage multiple projects from start to finish.
  • Capability to work independently and as part of a team.
  • Knowledge of the Homeless Management Information System (HMIS)/Wellsky.
  • Valid Arizona driver’s license; pass motor‑vehicle record check.
  • Successfully complete The Salvation Army’s Defensive Driving course.
  • Reliable transportation.
  • Pass criminal background check.
  • Minimum age 21.
  • Preferred: 6 months of Street Level Case Management experience.
Responsibilities
  • Develop and maintain a schedule to reach and engage individuals experiencing homelessness in the Tempe community.
  • Update and maintain a service and referral information list of community partners; build relationships, provide warm handoffs.
  • Conduct initial assessments to determine client needs.
  • Perform Coordinated Entry interviews; enter data into HMIS/Wellsky.
  • Maintain weekly contact with clients, building relationships and supporting their path toward stable housing, employment, education, or self‑support.
  • Organize, inventory, and order essential supplies.
  • Facilitate, organize, and train heat‑relief volunteers and staff.
  • Act as bridge between mobile and Corps programs.
  • Refer individuals requesting pastoral care to Corps Officers.
  • Provide reports on program success.
  • Perform other administrative and operational functions as requested.
  • Assist intake, screening, and implementation of material assistance opportunities; track funding and input data into Wellsky.
  • Track, analyze, and report data to county, donors, and internal stakeholders.
  • Coordinate services with agencies (mental health, employment, housing, substance abuse, vocational services).
  • Travel throughout Tempe and work in remote locations, even in extreme heat.
  • Engage community partners to maximize partnerships and resources.
  • Perform additional duties directed by Corps Officer related to the Street Level Program.
Physical Requirements

Qualified individuals must be able to perform the essential duties of the position with or without accommodation. The job requires walking, standing, climbing, balancing, stooping, kneeling, crouching, crawling, and reaching continuously. Ability to work in extremely hot summer weather and moderately cold winter temperatures. Must regularly lift up to 20 lbs, frequently lift up to 50 lbs, occasionally lift up to 75 lbs.

Vision requirements include close vision, distance vision, color vision, depth perception, and focus ability. Ability to operate a phone, desktop, or laptop computer and to produce written information.
