Manager Care Coordinator

Under the supervision of the Director Care Management, the Manager Care Coordinator will provide clinically oriented management of daily operation for the social services programming division. The manager will share 24/7 responsibility/accountability for the daily operations within the care management department among other department leaders across the continuum of care. This position will support the coordination of care for inpatients and outpatients within the system.

The manager uses depth and breadth of clinical management knowledge to ensure the provision of high quality, safe, and cost-effective patient care. The target population for services includes, but is not limited to, individuals with severe and persistent mental illness. This position will also deal with acute medically complex patients requiring intense care coordination to prevent a readmission, many of whom also experience lack of basic needs such as food, housing, and other resources.

Salary Pay Range: $83,116.80 - $

• Requires a Master's degree in Social Work.

• Must have a minimum of five (5) years of progressively responsible care management related experience that demonstrates a strong understanding of the required knowledge, skills and abilities.
• Requires prior supervisory/management experience, preferably within a healthcare or care management environment.

• Requires an LMSW (Licensed Master Social Worker) or LCSW (Licensed Clinical Social Worker).
• Prefer either CCM (Certified Case Management) or ACM (Accredited Case Manager).
• Requires BLS. A BLS card obtained through an approved American Heart Association (AHA) training center is preferred.
• Ambulatory Care Management:
  Must have a valid fingerprint clearance card issued by the Arizona Department of Public Safety OR submit a completed Affidavit at time of hire. Must complete fingerprint clearance card application within seven (7) days of hire date. Must present actual card within ninety (90) days of applying for fingerprint clearance card.

• Must have an awareness of and sensitivity to the cultural diversity of the population served.
• Requires the ability to read, write and speak effectively in English.

Hourly pay rate: $39.96 - $58.94
