• Conduct comprehensive assessments to identify emotional, social, and environmental factors affecting patient health.
• Develop and implement individualized care plans in collaboration with the healthcare team.
• Provide counseling and support to patients and families coping with illness, injury, or end-of-life issues.
• Connect patients with community resources and services to enhance their well-being.
• Maintain accurate and timely documentation in compliance with HIPAA and agency policies.

• Master’s degree in Social Work (MSW) from an accredited program.
• Current state licensure.
• Minimum of 1 year of experience in a healthcare or home health setting.
• Valid driver’s license and reliable transportation.
• Strong interpersonal and communication skills.
• Ability to work independently and manage a caseload effectively.
