Job Summary
Client Care Coordinator is a shelter or temporary lodging site‑based position whose primary role is to manage a housing caseload and coordinate care for individuals and families experiencing homelessness. The coordinator collaborates with the CBI Continuum of Care or community agencies to address complex substance use, mental health, housing, and medical needs, serving as the bridge between sheltered clients and community‑based services.
Responsibilities• Coordinate care through the CBI Continuum of Care (or other partner agencies) to address social determinants of health and facilitate housing and community integration programs.
• Assist clients in locating suitable housing units using housing search software and support their transition out of homelessness.
• Enter data into the established data system, assist leadership with ongoing reports, and document required interactions per AAC, ADHS/DBHS, CARF, and program policies.
• Produce end‑of‑shift reports summarizing coordination activities for each client.
• Work collaboratively with a multidisciplinary team—including patient care techs, registered nurses, counselors, and clinical leads—to ensure seamless delivery of care.
- High school diploma or GED required; associate degree (or higher) in a field related to Behavioral Health is preferred.
- Minimum of six months of recovery from substance use and/or mental health disorders.
- Minimum of six (6) months of paid related work experience in behavioral and medical crisis settings.
- One to three (1–3) years of full‑time health‑care related experience preferred.
