** Ahwatukee Post Acute is growing — and we are seeking a dedicated Physical Therapist to join our in-house therapy team! Our 192 bed facility is currently undergoing a major renovation, including construction of a state**
- ** of-the-art  44bed behavioral and dementiacare unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities.

** What You’ll Do
*** Perform skilled PT evaluations and provide evidence-based treatment
* Develop individualized care plans to increase function, safety, and mobility
* Document patient progress using Net Health
* Collaborate closely with nursing and IDT using Point Click Care
* Educate patients, families, and caregivers on therapy strategies
** You’ll Work With:
*** 1 other Full-Time PT
* Multiple Full-Time PTAs
* 2 Full-Time OTs & multiple Full-Time COTAs
* Full-Time onsite SLP  A strong and fully staffed in-house therapy department — no contracting or travel required
** Qualifications
* ** Arizona PT license (or eligible prior to start date)
* Strong communication and teamwork skills
* A passion for working with the geriatric and memory care population
