Physical Therapist (Full-Time) - Ahwatukee Post Acute
Skilled Nursing Facility | Phoenix, AZ
Compensation: $50-$55 per hour (based on experience)
Schedule: Full-Time | Monday-Friday
Optional alternative schedules:
Tuesday‑Saturday or Sunday‑Thursday
Ahwatukee Post Acute is growing — and we are seeking a dedicated Physical Therapist to join our in‑house therapy team! Our 192‑bed facility is currently undergoing a major renovation, including construction of a state‑of‑the‑art 44‑bed behavioral and dementia care unit, enhancing our ability to provide specialized services to the Phoenix and Ahwatukee communities.
We proudly maintain a CMS 5‑Star Quality Measure rating and foster a supportive, collaborative clinical environment focused on patient‑centered success.
What You’ll Do- Perform skilled PT evaluations and provide evidence‑based treatment
- Develop individualized care plans to increase function, safety, and mobility
- Document patient progress using Net Health
- Collaborate closely with nursing and IDT using Point Click Care
- Educate patients, families, and caregivers on therapy strategies
- 1 other Full-Time PT
- Multiple Full-Time PTAs
- 2 Full-Time OTs & multiple Full-Time COTAs
- Full‑Time onsite SLP
A strong and fully staffed in‑house therapy department — no contracting or travel required.
Qualifications- Arizona PT license (or eligible prior to start date)
- Strong communication and teamwork skills
- A passion for working with the geriatric and memory care population
