Position: Wellness Nurse / Nurse Med Tech (LPN)

Location: Phoenix, AZ

Shift: 6:00 A–6:00 P rotating shifts, every other weekend required.

• Monitor the health and well‑being of residents, providing leadership support and assisting with resident care and services.
• Administer medication and maintain clinical quality assurance.
• Communicate with resident support services, families, and maintain documentation.
• Leverage wellness elements core to Phoenix programs.
• Identify ongoing needs and services of residents through the assessment / PSP process.
• Ensure proper follow‑through and documentation for residents with changes in clinical care needs.
• Assist the Wellness Director with completing the clinical sections of monthly wellness visits and PSP.
• Inform the Wellness Director and/or Executive Director of any changes in medication or service level that may provoke a higher tier level or potential move‑out.
• Assist with obtaining weights and vital signs monthly for each resident prior to completing monthly wellness health updates.
• Assist with direct resident care as needed.

• Licensed Practical Nurse (LPN) or Registered Nurse (RN).
• Minimum of one year of experience working with elders or disabled individuals (preferred).
• Demonstrated leadership capabilities.
• Knowledge and experience in Assisted Living industry and Dementia care or Skilled Nursing (preferred).
• Ability to work weekends, evenings and flexible hours.
