Community Bridges, Inc. Lead NavigatorA Lead Navigator is an individual who is in recovery from AOD use and/or GMH for at least one year and has a minimum of 90 days experience as a Navigator or Peer Support Specialist. The primary role is to work with homeless individuals, providing outreach and supportive services to assist them in ending their homelessness. The Lead Navigator serves as the bridge for the patient, coordinating and monitoring behavioral health and community‑based services, and facilitating delivery of these services.
Responsibilities also include conducting outreach, engagement, case management, and ongoing wrap‑around services, mentoring Navigator II staff, and mastering coordinated entry, housing referral, leasing, wrap‑around services, and discharge processes as part of a multidisciplinary team.
- High school diploma or GED required. Associate degree or higher in a field related to behavioral health preferred.
- Recovery from AOD use and/or GMH for at least one year.
- 1–3 years of full‑time healthcare related work experience preferred, but not required.
- Equivalency of education and experience acceptable.
- Current AZ driver’s license and clear 39‑month motor vehicle record.
- Behavioral Health Technician certification (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI clinical policies.
- Arizona fingerprint clearance card, site‑specific.
