CORONADO HEALTHCARE CENTER – PHOENIX, AZ. Full Time Occupational Therapist. Flexible schedule with work-life balance. Pay: $40 - $50/hour based on experience. Bonus: $5,000 sign-on bonus.

• Effectively screens/evaluates patients with cognitive and/or ADL physical functioning disorders and develops an appropriate plan of care following all regulatory and clinical practice standards.
• Utilizes standardized assessments and evidence-based practice to support clinical interventions.
• Provides rehabilitative, skilled and medically necessary treatment interventions to patients with cognitive and/or ADL physical functioning disorders per regulatory and clinical practice requirements.
• Administers various procedures as part of the rehabilitation plan including ADL retraining; therapeutic activities; therapeutic exercises; modalities; use of supportive and assistive devices; and related therapies.
• Assesses patient progress and adjusts treatments accordingly in the clinical record per regulatory and clinical practice requirements.
• Evaluates treatment outcomes and modifies services based on patient evaluation and referrals.
• Provides consultation and counseling to patients, families, caregivers and other service providers related to physical disorders.
• Effectively communicates with supervisor and health team members regarding patient progress, barriers, and treatment plans.
• Provides comprehensive discharge summaries of services per regulatory and clinical requirements.
