• Provide social work services to facilitate solutions to medically related psychosocial and environmental problems of patients and families.
• Ensure continuity of care by implementing interventions, including initial assessment, identification of mental health disorders, brief behavioral health interventions, information and referral during hospitalization and/or outpatient treatment.
• Offer educational support to staff.
• Maintain positive working relationships with vendors and external agencies to access services for patients.
• Clinically assess and treat emotional mental health issues, under the direct supervision of an LCSW social worker.
• Assess data reflective of patient status and interpret information relative to the patient’s age‑specific needs.

• Master’s degree in Social Work.
• One year of current experience in a clinic, hospital, medical setting, or healthcare field (preferred).
• Knowledge of adjustment to medical diagnosis and chronic disease.
• Strong communication skills to interact with patients, family members/caregivers, providers, and allied health staff.
• Psychosocial diagnostic skills and ability to coordinate transition of care.
• Active license through Arizona State Board of Behavioral Health Service Examiners;
  Licensed Master Social Worker (LMSW) required.
