ACT Rehabilitation Specialist - 51st Clinic

Join Terros Health as an ACT Rehabilitation Specialist in our 51st clinic in Phoenix, AZ.

Primary hours at the site:
Mon-Fri 7:30 AM – 6:00 PM. Flexible schedule required as this team provides treatment 7 days/week, including weekends and holidays.

Pay range: $18.38 – $25.00 per hour (DOE). $3/hour ACT Differential in addition to base pay. Full‑Time, Employed.

Location:

4616 N 51st Ave., #108 in Glendale, AZ; approximately 20% in an office setting, 80% in the field.

Qualifications:

• Bachelor's Degree in Behavioral Health (preferred). No degree requirement, but experience must be demonstrated.
• Must be bilingual – bilingual differential available.
• Must have a very flexible schedule to provide treatment 7 days/week, including weekends and holidays.
• On‑call and mileage reimbursement provided.

Responsibilities:

• Provide direct treatment, rehabilitative and supportive services to clients served on the ACT team.
• Assess members' needs and wants, participate in the development of individual service plans.
• Act as a client advocate and liaise with families and community agencies.
• Serve as the team's specialist for rehabilitation services by collaborating with other team members in assessing and delivering rehabilitation skills training as specified in the service plan.
• Provide on‑call crisis intervention services, including weekend and holiday coverage.
