Clinical Manager (RN )Home Health - (Pennsylvania License Required)
Location:
Philadelphia, PA (On-site, In-Office)
Job Type: Full-Time/Day shifts- mon-fri
Pay- up to $120K annually DOE
Position Summary:
We are seeking an experienced and compassionate Registered Nurse (RN) to join our leadership team as a Clinical Manager.
Key Responsibilities:
- Clinical Leadership
- Support and guide clinical teams to ensure delivery of high-quality patient care.
- Develop, maintain, and implement policies and procedures aligned with CMS, Medicare, and accreditation standards (ACHC, CHAP, Joint Commission).
- Regulatory Compliance & Quality Assurance
- Oversee clinical audits, licensing reviews, and survey readiness.
- Ensure compliance with all state, federal, and accrediting body regulations.
- Lead corrective action planning and performance improvement initiatives based on HHCAHPS and clinical KPIs.
- OASIS & PDGM Oversight
- Monitor OASIS documentation for accuracy and compliance.
- Implement effective PDGM strategies to optimize patient outcomes and reimbursement.
- Collaborate with leadership, clinical, and QA teams to ensure documentation integrity.
- Team Development & Recruitment
Qualifications: - Active RN license in Pennsylvania (Required)
- BSN or MSN preferred
- Minimum 5 years of experience in a Medicare-certified home health agency
- Proven expertise in PDGM, OASIS, CMS regulations, and home health operations
- Familiarity with ACHC, CHAP, or Joint Commission accreditation standards
- Experience with EMR systems (Alora experience a plus)
