• Active Physical Therapist (PT) license in Pennsylvania

• Strong communication and interpersonal skills
• Ability to work independently and as part of a multidisciplinary healthcare team

• Evaluate, plan, and implement individualized physical therapy treatment programs for long-term care residents
• Collaborate with nursing staff, physicians, and other healthcare professionals to optimize patient outcomes
• Document patient progress and maintain accurate clinical records in compliance with healthcare regulations
• Educate and support residents and their families regarding therapy goals, exercises, and rehabilitation plans
• Participate in interdisciplinary team meetings and contribute to patient care planning
