As a Primary Care Physician, you will provide high-quality, outpatient care in a value-based care model, focusing on building long-term relationships with patients while driving positive health outcomes.
Practice Highlights:
100% outpatient primary care
Patient population: seniors with 3 5 chronic/complex conditions (e.g., heart disease, diabetes, hypertension)
Average of patients per day
Patient panel capped at 650
Dedicated care team including MA, scheduler, referral specialist, and full leadership support
Medical, dental, and vision coverage (family options available)
