Position: Integrated Care Nurse Practitioner (Hybrid)

Overview

Position: Integrated Care Nurse Practitioner (Hybrid) •
Schedule: Monday–Friday, 8:00 AM–5:00 PM •
Hybrid Role: 1–2 days telehealth (remote) + 3–4 days in patients' homes (varies weekly)
• No Weekends | No On-Call •
Compensation: Base salary +
15% quarterly bonus
Perks: Mileage reimbursement + company cell phone •
Sign-On Bonus Available

Overview:
We are seeking compassionate and skilled Nurse Practitioners and Physician Assistants to join an integrated care team focused on improving quality of life for patients with chronic illness, including kidney disease. This hybrid role combines in-home visits and telehealth, allowing providers to deliver personalized, evidence-based care where patients need it most.

You will work alongside a Registered Nurse and Social Worker to reduce avoidable hospitalizations, close gaps in care, and support long-term patient health outcomes.

Responsibilities
  • Conduct comprehensive assessments and annual wellness visits (home and virtual)
  • Develop and manage treatment plans; prescribe medications and order diagnostics
  • Coordinate care with primary care providers, specialists, and ancillary services
  • Educate patients and families on disease management and preventive care
  • Maintain timely, accurate clinical documentation
  • Participate in integrated care team meetings
  • Deliver care focused on quality outcomes, patient satisfaction, and prevention
Requirements
  • Active, unrestricted NP or PA license
  • Board certified (NP: ANCC/AANP | PA: NCCPA)
  • Active DEA license
  • 2+ years of direct patient care experience
  • Experience with annual wellness visits (home or virtual preferred)
  • Ability to practice independently and manage a patient panel
  • Valid driver s license, reliable transportation, and insurance
  • Managed Care / IPA / Health Plan experience preferred
