Overview
The Primary Care Access and Referral Program is seeking (3) Full Time Primary Care Access & Referral Specialists (Office Assistant Seniors) to serve in the newly reorganized Unit. Two of the positions are Bi-lingual Spanish required. The remaining position does not have the language requirement. The purpose of these positions is to support the timely and effective appointing of underserved, low income Multnomah County residents into a primary care “home” for comprehensive care and into specialty care through comprehensive, collaborative, planned processes that are patient centered, respectful and attentive to resource stewardship.
The PCAR Specialist will contact community organizations, patients and referred community clients in acquiring and providing information and making referrals. These team members will provide a wide variety of advanced clerical work, including the maintenance of records, application of departmental guidelines and standards, and will research, complete and analyze data related to patient referrals into the department’s primary care system and from Multnomah County Health Department’s Primary Care System into specialty care throughout the metropolitan area.
AboutThe Program
The Primary Care Access and Referral (PCAR) Program appoints underserved, low income, uninsured individuals into Multnomah County Health Department (MCHD) Primary Care services and refers from MCHD Primary Care to other community providers through standardized referral processes. Those individuals referred through PCAR will be assigned a Primary Care medical home for comprehensive care. PCAR will collaborate with county organizations that also provide services to underserved individuals and internal service providers, as well as with Corrections Health and other MCHD providers.
PCAR will ensure comprehensive, collaborative planning processes that are patient centered, respectful and attentive to resource stewardship.
Experience: Three years of specialized, increasingly responsible clerical experience in the area of assignment.
Training: Equivalent to the completion of the twelfth grade. Additional specialized clerical training or training in business services/operations is desirable.
Other: Applicants must be current regular or limited duration employees of the Health Department’s A.I.C. Unit.
