Overview

We are currently looking to recruit an enthusiastic and motivated Occupational Therapist to join the Peterborough Community Rehabilitation Service based at Botolph Bridge Community Health Centre, Peterborough. This role is community based covering Peterborough and the surrounding area. You will be working as part of a multidisciplinary therapy team providing high quality rehabilitation to patients with physical disabilities in their home environment.

Please note for this role you will be required to travel independently around the county meeting strict time deadlines. You will need to hold a full UK driving licence
* and have use of a vehicle. You cannot use public transport for this role as this is not a reliable form of transport and will not allow you to meet service needs. Please confirm in your application that you meet the specified criteria.

Responsibilities
  • Assess, plan and develop specialist occupational therapy programmes for patients with a wide variety of complex needs.
  • Manage your own caseload, prioritising referrals according to clinical need, signposting appropriately and undertaking assessment of patients with complex and multiple pathologies using advanced, specialist clinical reasoning skills.
  • Supervise junior staff and students, oversee patient intervention and ensure that Therapy Assistants deliver high quality patient care.
  • Provide specialist advice to all members of the multi-disciplinary team, service users and appropriate others on changes to current care plans, patient progress and other relevant matters to promote independence and reduce hospital admission where possible.
  • Organise and manage your own time, delegating work appropriately.
  • Contribute to the development of the service/organisational aims and objectives of the Community Rehabilitation Service and the new model of care.
  • Attend MDT Case reviews as required to assess patient understanding of treatment proposals and to work within a legal framework with patients who lack capacity to consent.
Requirements/Qualifications
  • Full UK driving licence and use of a vehicle; willingness to travel around the county and meet service needs.
  • Autonomous practitioner within an Integrated Multi Professional Community Rehabilitation Team; able to provide high standard of care to patients.
  • Ability to undertake holistic assessments (including those with complex presentations and multi-pathologies) using specialist clinical reasoning skills.
  • Excellent communication skills to work effectively with patients, carers, colleagues and other health care professionals; ability to refer to other services as appropriate.
  • Commitment to evidence-based practice and contribution to service development.

You will support the Clinical Lead with day to day planning and delivery of a high quality care, using evidence-based practice, and conduct risk assessments and health and safety assessments, including immediately reporting any changes/newly identified risks. You will be expected to organise and manage your own time, delegating work appropriately. Your role will be actively involved in the development of the service/organisational aims and objectives of the Community Rehabilitation Service and the new model of care.
