River Mead is seeking a compassionate and experienced Nurse to serve as a Nursing Coordinator for our Memory Support neighborhood in Peterborough. This leadership role combines hands‑on clinical care with team coordination to ensure residents living with memory loss receive exceptional, person‑centered support.
The Nursing Coordinator plays a key leadership role by guiding and supporting Nurses, MNAs, LNAs, and PCAs across all shifts. This position ensures continuity of care, promotes strong communication among interdisciplinary teams, and helps maintain a safe, resident‑centered environment.

- Conduct resident assessments and provide specialized nursing care within the Memory Support neighborhood.
- Develop, implement, and update individualized care plans in collaboration with residents, families, and the interdisciplinary team.
- Administer medications and treatments, including wound care, immunizations, and infection management.
- Monitor resident conditions and communicate changes with physicians and healthcare providers to ensure timely care.
- Lead and support nursing and direct care staff while overseeing daily clinical operations and ensuring regulatory compliance.
- Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in the state of New Hampshire.
- 2–3 years of clinical nursing experience preferred, ideally in memory care, assisted living, or long‑term care.
- Experience working with individuals living with dementia or cognitive impairment strongly preferred.
- Demonstrated leadership ability or experience coordinating care teams.
- Strong clinical assessment, care planning, and problem‑solving skills.
- Excellent communication and collaboration abilities.
