The Licensed Practical Nurse (LPN) is responsible for monitoring the health and well-being of residents while providing leadership support and assistance with resident care and services. This role supports the overall well-being of residents, assists with medication administration oversight, communicates with resident support services and families, and helps maintain clinical quality assurance in accordance with federal, state, and local standards to promote the highest degree of quality care and services.

- Ensure proper follow-up, assessment, and documentation for residents experiencing changes in health care status or services.
- Assist the Director of Nursing (DON) with resident assessments at move-in, 30 days post move-in, every six months, and with any change in condition.
- Conduct quarterly care plan updates and complete nursing progress notes summarizing changes.
- Assess and monitor residents following falls or other incidents; document and report as required.
- Maintain frequent and timely communication with residents’ families or responsible parties regarding medical needs.
- Contact residents’ primary care providers when necessary or upon family request.
- Ensure monthly weights and vital signs are obtained for each resident.
- Assist with maintaining health records in accordance with policies and regulatory requirements.
- Support the Director of Nursing in preparation for state surveys.
- Order medications and medical equipment as needed.
- Communicate resident changes, concerns, and solutions to the Director of Nursing as appropriate.
- Monitor resident medication and treatment profiles to ensure accurate administration and documentation.
- Promptly fax new medication orders to the pharmacy and enter them into the Medication Administration Record (MAR).
- Maintain cleanliness, organization, and compliance within the medication room, including monitoring for expired medications.
- Maintain confidentiality of all resident information.
- Follow established infection control practices.
- Report occupational exposures to blood, body fluids, infectious materials, sharps, or hazardous chemicals immediately.
- Ensure oxygen tanks are stored safely, exchanged when empty, and monitored to ensure prescribed flow rates.
- Attend required meetings, including town halls, department meetings, care plan meetings, and others as directed.
- Graduate of an approved college or school of nursing –
Required. - Current state license as a Licensed Practical Nurse –
Required. - Minimum of one (1) year of experience in home health, assisted living, hospital, or long-term care –
