ResponsibilitiesUtilizing a collaborative process, the Case Manager will establish a continuum of care plan for a caseload of patients. The Case Manager will establish quality monitoring indicators, collection of data, analysis, documentation and the reporting outcomes, and liaison with patients. The Case Manager will act as a liaison to facilitate a smooth transition for the patient from acute to the next level of care.
QualificationsRequired Qualifications
- Upon hire:
California Registered Nurse License - 2 years Acute care clinical or comparative experience.
- Bachelor’s Degree in Nursing. Required within 3 years of hire date.
- Upon hire: CCM
- 3 years experience in community health nursing or case management.
