Base pay range: $130,000 – $160,000 per year, plus up to $10,000 annual incentive bonus.

All positions require in‑home patient visitation. We are actively expanding into Marin and Sonoma counties.

• Develop and implement care plans to promote wellness and prevent re‑hospitalization.
• Teach patients and family/caregivers wound care, blood draws, tube feeding, tracheostomy care, Foley catheter care, CHF management, and medication education.
• Perform a full assessment of patient and family during the initial visit and use data to develop advanced care plans including rehabilitation.
• Collaborate one‑on‑one with patients and family to achieve improvement and/or stability.
• Case‑manage care episodes with interdisciplinary clinical team.
• Prepare clinical notes, orders, and charting in a compliant time frame.
• Collaborate with physicians and agency staff to deliver best care possible.
• Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with protocols.

• Current unrestricted California RN license
• Current BLS/CPR certification
• Valid California driver’s license
• High level of clinical knowledge in home health care (preferred)
• Experience with high‑acuity patients (plus)
• Experience working with interdisciplinary teams (highly desired)
• Case management fundamentals (highly desired)
• Previous experience with IV medication infusions (highly preferred)
• Proficiency in initiating intravenous therapy (plus)
• Self‑directed, empathetic, critical thinker with outstanding interpersonal and leadership skills
