Base pay range: $130,000 – $160,000 per year, plus up to $10,000 annual incentive bonus.
Job OverviewAll positions require in‑home patient visitation. We are actively expanding into Marin and Sonoma counties.
Responsibilities- Develop and implement care plans to promote wellness and prevent re‑hospitalization.
- Teach patients and family/caregivers wound care, blood draws, tube feeding, tracheostomy care, Foley catheter care, CHF management, and medication education.
- Perform a full assessment of patient and family during the initial visit and use data to develop advanced care plans including rehabilitation.
- Collaborate one‑on‑one with patients and family to achieve improvement and/or stability.
- Case‑manage care episodes with interdisciplinary clinical team.
- Prepare clinical notes, orders, and charting in a compliant time frame.
- Collaborate with physicians and agency staff to deliver best care possible.
- Initiate and administer IV medications (antibiotics and diuretics) infusions safely and effectively in accordance with protocols.
- Current unrestricted California RN license
- Current BLS/CPR certification
- Valid California driver’s license
- High level of clinical knowledge in home health care (preferred)
- Experience with high‑acuity patients (plus)
- Experience working with interdisciplinary teams (highly desired)
- Case management fundamentals (highly desired)
- Previous experience with IV medication infusions (highly preferred)
- Proficiency in initiating intravenous therapy (plus)
- Self‑directed, empathetic, critical thinker with outstanding interpersonal and leadership skills
