Base pay range: $/yr - $/yr

In partnership with the Case Manager you will provide skilled services to assist patients with breathing, speaking, and swallowing functions. You will evaluate and treat speech, language, cognitive and fluency disorders that affect communication. You will serve as an educator, patient advocate and a member of the interdisciplinary care team. Territory includes Solano County and Petaluma/Sonoma County area.

• Current unrestricted CA Speech Therapist License
• Current BLS/CPR Certification
• Valid CA Driver's License
• Knowledge of development and human behavior as it relates to illness/injury; understanding of how culture influences health care
• Knowledge of state and federal homecare regulatory guidelines preferred
• Home health care experience highly preferred; experience with high-acuity patients is a plus
• Experience working with an interdisciplinary team; case management fundamentals are highly desirable
• Self-directed, empathetic, critical thinker with outstanding interpersonal and leadership skills; organized

Anticipated wage range:
Speech Therapist: $ - $. Starting pay is commensurate with relevant experience. 21st Century offers generous bonus incentive plans and 401(k) with up to 4% match.
