The LPN/RN is responsible for delivering high-quality clinical care while supervising and supporting nursing assistants. This role ensures residents receive safe, effective, and individualized care in accordance with physician orders, care plans, and regulatory standards.

• Administer medications and treatments safely and accurately
• Assess residents and promptly report changes in condition
  
  Supervise, mentor, and support CNAs
• Maintain accurate and timely documentation
• Communicate effectively with residents, families, physicians, and the interdisciplinary team
• Promote a safe, positive, and welcoming environment
• Ensure compliance with all state and federal regulations

• Active Illinois LPN or RN license (in good standing)
• Current CPR certification

Long-term Care Experience Preferred But Not Required
