Responsibilities
  • Collect comprehensive data pertinent to the patient's health or the situation.
  • Perform age and disease specific assessment/data collection independently in a systematic manner focusing on physiologic, psychological and cognitive status.
  • Recognize clinical and diagnostic status changes in patient status.
  • Identify variables in patient/family education needs based on age, disease, and culturally specific learning differences.
  • Develop a plan of care that prescribes strategies and interventions to attain expected outcomes.
  • Formulate age and disease specific patient/family centered goals/outcomes based on assessment data.
  • Develop and coordinate a plan of care which is patient/family focused, prioritized and encompasses the continuum of care.
  • Communicate, coordinate, and implement the plan of care.
  • Provide ongoing communication with patient/family, nursing colleagues and other members of the health care team regarding patient status, rendered care and treatment plans.
  • Implement age and disease specific care in an organized and timely manner.
  • Provide or coordinate the patient/family education as needed and ensure understanding.
  • Perform according to established protocols in clinical situations, including communicating with appropriate persons and documenting events.
  • Provide nursing care and treatments that reflect the patients' needs, advocate appropriately and demonstrate evidence-based nursing practice and compliance with standards.
  • Perform continuous point‑of‑care electronic documentation of each individual patient’s assessments, plan‑of‑care, interventions and evaluation of rendered care.
  • Administer medication utilizing the newest advancements in bar‑scanning and documentation technologies designed to ensure the safest delivery of medications to our patients.
  • Engage in multidisciplinary discharge of patients to ensure safe transitioning and continuum of care.
  • Evaluate progress toward attainment of outcomes.
  • Utilize critical‑thinking skills based on nursing knowledge to achieve desired patient outcomes.
  • Initiate interventions related to patient’s change in condition and unexpected response to care.
Additional Responsibilities
  • Identify ethical issues in the practice setting and bring them to the attention of other team members.
  • Recognize and respond to opportunities to enhance patient satisfaction.
  • Maintain competencies, certifications and continuing education to meet facility accreditation or licensing requirements.
  • Adhere to the American Nurses Association Code for Nurses with Interpretive Statements.
  • Lift a minimum of 35 lbs., push and pull a minimum of 10 lbs. and stand a minimum of 6 hours a day.
  • Adhere to HMH's Organizational and Managerial competencies and standards of behavior.
Qualifications
  • BSN in progress and/or willing to acquire within three (3) years of hire (for hires on or after November 2019).
  • At least six (6) months of relevant nursing experience OR active participation in Hackensack Meridian Health's Nurse Residency Program.
  • Strong ability to learn quickly, adapt to changing patient needs, and a dedication to ongoing education.
  • Exceptional teamwork, accountability, and commitment to delivering the highest quality of care.
Licenses and Certifications Required
  • NJ State Professional Registered Nurse License.
  • AHA Basic Health Care Life Support (HCP) Certification within 60 days of entering position.
Compensation

Minimum rate of $45.73 hourly. This is a good‑faith estimate of the minimum base pay for this role at the time of posting in accordance with the New Jersey Pay Transparency Act. Actual pay may vary based on market…
