Full‑time position. Competitive pay: $30.60–$40.20 per hour.
Work schedule:
Monday–Friday, 8 am–5 pm, one day off mid‑week. Approximately 32 hours per week with 10 paid holidays, 4 weeks of PTO per year, 401(k) match up to 4%, and health benefits (medical, dental, vision) starting Day 1.
Position Summary
The Registered Nurse Case Manager coordinates and facilitates plans of care, including education, support, clinical care, and care management for individuals, aligning with Heartland Health Services’ mission, strategic goals, federal and state laws, accreditation standards, and performance objectives.
Essential Functions
- Conduct patient assessments and coordinate care and communication among multiple clinicians.
- Support patients in establishing care plans, self‑management goals, and community resource linkages.
- Maintain individual patient care plans, identifying, coordinating, monitoring, and evaluating services to ensure continuity of care for assigned case load.
- Eliminate fragmentation of patient care while effectively utilizing organizational resources and support systems.
- Ensure proper use of equipment and supplies; practice safety, environmental, and infection‑control methods.
- Complete all clinic and program documentation accurately and in a timely manner.
- Pursue professional growth and maintain competencies in nursing care management.
- Support clinic compliance with all applicable rules, regulations, protocols, and procedures.
- Participate in continuous quality improvement efforts to improve patient outcomes.
- Collaborate with clinical teams, direct and indirect support staff to develop and implement patient‑centered policies and procedures.
- Maintain confidentiality of patient information per HHS policies.
- Perform clerical duties or department tasks as assigned.
- Attend all staff and department meetings and any other required gatherings.
Requirements
- Current Registered Nurse license.
- 3–5 years of experience in community health, public health, chronic disease management, or community nursing.
- Experience in care coordination preferred.
- Ability to work independently and manage a large, fluctuating workload.
- Capability to care for medically complex patients.
- Strong teamwork skills, interfacing with patients, primary care clinicians, support staff, and community agencies.
- Excellent organizational and time‑management skills.
- Strong verbal and written communication skills.
- Computer proficiency with electronic health record systems required.
- Basic Life Support certification: not required to start but must obtain within first 90 days.
