Role DescriptionThis is a full-time, on-site role for a Wound Care Provider located in Peoria, IL. The Wound Care Provider will be responsible for conducting advanced wound care consultations, developing individualized patient care plans, performing procedures as required, and regularly assessing patient progress. Additional duties include collaborating with facility staff to enhance their wound care skills and implementing preventative care strategies tailored to the needs of the setting.
Qualifications- Strong clinical skills in wound care, including the ability to conduct wound evaluations, treatments, and procedures effectively
- Knowledgeable in creating and managing patient care plans, with attention to detail and adherence to clinical protocols
- Collaboration and communication skills to work closely with facility staff and multidisciplinary teams
- Commitment to continuous professional development and staying updated on the latest wound care advancements
- Valid medical licensure and certifications relevant to wound care practice
- Experience in post-acute care settings, particularly in skilled nursing or long-term care facilities, is a plus
