The Family Medicine Nurse Practitioner will provide comprehensive primary care services in local nursing homes and manage care for elderly and long‑term care populations.
Responsibilities- Deliver primary care services including health assessments, diagnosis, treatment planning, and disease prevention
- Manage chronic and acute health conditions in collaboration with supervising physicians
- Provide continuity of care by rounding at local nursing homes on a scheduled basis
- Order and interpret diagnostic tests and lab work
- Educate patients and families on treatment plans and preventive care
- Document all patient interactions in the EHR
- Coordinate care with other healthcare professionals and facilities
- Current Nurse Practitioner license in the State of Illinois
- National certification as a Family Nurse Practitioner (FNP‑C or FNP‑BC)
- Minimum of 1–2 years of experience in family medicine or primary care preferred
- Experience or interest in geriatric care and nursing home rounding
- Strong communication, critical thinking, and organizational skills
- Ability to work both independently and as part of a multidisciplinary team
- Salary Range: $100,000–$140,000 annually (Collections Based Pay Structure)
