Position Purpose

As a member of the therapy team, the Physical Therapist’s primary function is to provide a thorough therapy evaluation, diagnosis, a detailed plan of care including treatment methods and procedures; supervision and guidance of assistants and consultation with referral sources and other healthcare providers.

Requirements
  • Licensed or eligible for licensure by the State of Illinois under the Illinois Physical Therapy Practice Act.
  • Member or eligible for membership in the APTA preferred.
  • General knowledge and therapy skill to perform the evaluation and treatment of a wide variety of conditions; and ages from neonates, pediatrics, young adults, and adults to geriatrics.
  • Current certification in CPR and other service line specific competencies.
Essential Job Functions
  • Perform examinations and evaluations; establish a Physical Therapy diagnosis and treatment plan.
  • Provide evidence‑based Physical Therapy treatment.
  • Documentation, reporting, and information dissemination.
  • Case management functions: evaluate effectiveness, revise the plan, and discharge from services.

  • Demonstrate interpersonal skills by making customers feel welcome and respected as individuals; protecting their privacy and confidentiality.
