* This is a full time position that will service the North Phoenix area (Cave Creek, Carefree, North Scottsdale) OR the West Phoenix area (Glendale, Peoria).
* $80000 - $90000 per year (depending on experience), plus travel expenses.

What You'll Do

As a Home Health RN, you'll be the heart of patient care, bringing clinical expertise and compassion directly into the home. You will:

* Build meaningful relationships with patients and families

* Create personalized care plans that truly make an impact

* Coordinate care as a trusted Case Manager

* Educate and empower patients and caregivers with confidence

What We're Looking For

* Current RN license in the State of Arizona

* Valid driver's license and reliable, insured vehicle

* Strong clinical skills and a passion for patient-centered care

* Ability to work independently and make a real difference
