As an LVN with Remedy Home Health, you will deliver hands‑on skilled nursing care to patients in their homes under the supervision of a Registered Nurse (RN).
Primary Responsibilities- Provide skilled, patient-centered nursing care based on individualized care plans.
- Accurately document all visits, assessments, and treatments using electronic health records (EHR).
- Communicate effectively with patients, families, and the home health care team.
- Ensure compliance with Arizona home health regulations and company policies.
- Help patients meet their health goals through compassionate and professional nursing.
- Current Arizona Licensed Vocational Nurse (LVN) license in good standing.
- Graduate of an accredited vocational nursing program
. - Minimum 1 year of nursing experience (home health experience preferred).
- Physical ability to assist and move patients as needed.
- Strong communication and documentation skills.
- Proficiency with electronic charting and care coordination software
. - Valid driver’s license, reliable transportation, and ability to travel to patient homes in Sun City and surrounding Maricopa areas
