Hospice Aide (CNA/LNA) (Healthcare) Low Caseload, Patient Focused:

Our Hospice Aide (CNA/LNA) Will:
- Work independently and take initiative to manage a caseload.
- Have autonomy and flexibility in managing their schedule.
- Utilize varying skills as a member of the clinical team as well as the community relations team.
- Develop meaningful, effective, and professional relationships with communities, families, and patients.
- Provide patients assistance with personal care including, but not limited to, baths, skin care, back rubs, oral hygiene, hair care, dressing, meal assistance, and light housekeeping.
- Assist in supporting the patients psychosocial wellbeing through companionship.
- Assess and report on patients condition and significant changes to the RN Care Manager.
- Assist with community relations events and activities including Vitals Clinics, seasonal activities, education sessions, meet and greets, etc.
