This position is offering a one-time sign-on bonus of $1500. Additional incentive bonus after 6-months, based on performance and other factors of $1500.

This is an in-office position based in Plymouth Meeting, PA.

The Lead Case Manager will be responsible for overseeing case management and case support specialist staff to ensure adherence with job responsibilities and agency expectations while managing an assigned caseload. This position will also be responsible to build and/or recommend the organization’s cultural education program to better assist clients in adjusting to life in the United States.

This position is expected to function effectively with moderate supervision while following the guidelines given on procedures, along with agency, federal, and state regulatory requirements.

ESSENTIAL JOB RESPONSIBILITIES
  • Maintain a caseload of clients in compliance with contract terms and agency expectations (approximately up to 10 cases).
  • Provide functional supervision to support staff and case managers as needed.
  • Update program documents, forms and manuals as needed.
  • Recommend and implement new trainings for case management and case support teams as needed.
  • Participate in clients file reviews as needed.
  • Partner with the Program Supervisory team in assessing fidelity to policy implementations on an ongoing basis of case management and case support teams.
  • Attend supervision meetings with case management and support staff as needed to ensure adherence to the expected job responsibilities, provide feedback, and lend support.
  • Provide case management coverage support within the program, as needed.
  • Assist with the implementation of all corrective action plans (CAP) by conducting CAP activities for case managers at meetings.
  • Oversee the case support and case managers training completion compliance in accordance with Bethany Christian Services, Department of Health and Human Services (DHHS), Office of Refugee Resettlement (ORR) and external stakeholders.
  • Effectively promote the Sanctuary Model of Care within case management and case support teams.
  • Stay abreast of all agency, federal Council on Accreditation (COA), and state regulatory requirements related to social services.
  • Promote a culture that aligns with Bethany’s mission, vision and values.
  • Perform other duties as assigned.
QUALIFICATIONS
  • Bachelor’s degree in Human Services, Behavioral Sciences, Social Services or related field of study from an accredited college.
  • At least three (3) years of progressive employment experience that demonstrates supervisory and case management experience.
  • Bi-lingual English and a second language, strongly preferred.
  • Demonstrated ability to service a diverse group of clients, to include refugees or other minority families.
  • Demonstrate leadership skills in alignment with agency and program values, while also exhibiting a team-player attitude.
  • Excellent verbal and written communication skills.
  • Demonstrated clinical, therapeutic, and crisis intervention skills.
  • Demonstrated ability with providing compassionate customer service.
  • Ability to work independently and exercise a high level of confidentiality.
  • Adheres to all agency and departmental safety procedures including reporting any unsafe practices, equipment and environment, and takes an active role in correcting the unsafe practice, equipment or environment through proper notification…
