Case Manager – Elkhart
Oaklawn Psychiatric Center is the leading provider of mental health and addiction services in Elkhart, Goshen, South Bend, Mishawaka and the surrounding communities of northern Indiana. We serve 27,000 children, adolescents and adults annually through a continuum of care ranging from skills training and case management to outpatient therapy and inpatient care, specializing in complex cases.

Case Managers help youth, adults, and families in our community live healthier, more fulfilling lives by identifying and supporting their needs and goals.
Primary Duties- Evaluating the consumer’s need for services
- Developing and monitoring a treatment plan that supports the consumer’s goals
- Assisting consumers to obtain services such as food, housing, medical care, Social Security benefits, transportation, skills training, etc.
- Following up to ensure services are being delivered and effective
- Advocating for families and serving as a link between DCS, court, schools, healthcare providers, employers, community agencies and other Oaklawn services
- Documenting all consumer interactions electronically
- Interacting directly with consumers (face‑to‑face; frequency varies with need and availability)
- Collaborating with outpatient therapists, physicians, other staff, community agencies and referral sources
- Managing caseloads that consider training, case complexity, frequency, and other duties
- Hours include availability in evenings and/or weekends
- Work directly with probation and Department of Child Services
- Parent education and supervised visitation
- Provide high‑fidelity Wraparound services
- BA/BS in social service or a related field and at least two years of experience in a social service setting; alternatively, an associate degree plus two years of direct casework with children and families
- Higher starting pay based on qualifications
- Valid driver’s license and proof of insurance
