We are seeking a dedicated and compassionate Hospice Nurse Practitioner (NP) to join our interdisciplinary hospice team. This role focuses on conducting Medicare-required face-to-face encounters, completing palliative consultative visits, and supporting patients with terminal illnesses through expert symptom management and goals-of-care discussions.
The ideal candidate is experienced in hospice or palliative care, knowledgeable in Medicare Conditions of Participation (CoPs), and comfortable collaborating with physicians and healthcare teams to ensure patients receive high-quality, patient-centered end-of-life care.
Responsibilities- Perform Medicare-required face-to-face visits in collaboration with the Medical Director
- Confirm terminal prognosis and hospice eligibility according to LCD guidelines and Medicare regulations
- Document clinical findings accurately and communicate recommendations for certification
- Provide expert pain and symptom management for patients with terminal illnesses (CHF, COPD, dementia, and related conditions)
- Conduct goals-of-care discussions, including POLST conversations
- Collaborate with attending physicians regarding treatment and symptom control
- Serve as a clinical resource for the interdisciplinary team
- Educate staff, patients, families, and community providers about hospice and palliative care
- Participate in performance improvement initiatives
- Act as a liaison between hospice and the medical community
- Support ethical decision-making processes
- Facilitate referrals to community providers when needed
Required:
- Active, unrestricted Pennsylvania Nurse Practitioner license
- Board Certification as a Nurse Practitioner (ANCC or AANP)
- Experience managing patients with terminal diagnoses
- Working knowledge of LCD guidelines for non-cancer hospice eligibility
- Strong clinical documentation and assessment skills
- Experience leading goals-of-care conversations
- Certification in Hospice & Palliative Care and/or Geriatrics
- Previous hospice or palliative care experience
- Strong clinical judgment
- Compassionate bedside manner
- Excellent communication and collaboration skills
- Ability to educate healthcare professionals and families
- Knowledge of Medicare Conditions of Participation
