Position SummaryThe Admissions Nurse (AN) is responsible for providing patient-centered care that is safe, efficient, timely, and effective. The AN collects and documents a systematic and comprehensive admission assessment of the patient’s condition using evidence-based assessment skills and techniques. Integrating psychosocial, emotional, and physical assessment findings, the AN collaborates with the admitting provider and interdisciplinary team to establish initial care goals and a treatment plan.
The AN uses Safe Crisis Management (SCM) and trauma-informed approaches to ensure a safe and supportive environment during all patient interactions.
- Demonstrates subject matter expertise in behavioral health assessment, physical nursing assessment, and nursing interventions.
- Understands the therapeutic milieu and interdisciplinary approach to care (psychiatry, medicine, nursing, social work, counseling, activities, pharmacology).
- Introduces self to patients, educates them on hospital and unit structure, and explains the role of each care team member.
- Completes comprehensive admission assessments using evidence-based skills and techniques.
- Collaborates with the admitting provider and team to develop initial care goals and a treatment plan.
- Completes all admission-related documentation, including pain, skin, and withdrawal assessments; nursing admission and treatment plans; and suicide assessments.
- Obtains provider orders, ensures medication reconciliation, and collects necessary lab and diagnostic specimens.
- Ensures all consent forms are completed and properly signed, and provides patients with the Patient Handbook.
- Performs hand-off communication using the SBAR tool to the receiving RN.
- Ensures belongings are searched and inventoried per policy; conducts or assists with patient body searches in accordance with policy.
- Administers STAT medications safely, verifying two patient identifiers and scanning barcodes.
- Monitors and documents patients’ responses to treatment, reporting changes promptly to the interdisciplinary team.
- Utilizes Safe Crisis Management (SCM) techniques and other approved de-escalation strategies to prevent and manage behavioral escalation.
- Communicates professionally and effectively with patients, families, and staff.
- Participates in performance improvement activities to enhance admission services.
- Provides assistance to other nursing units as directed when admissions volume is low.
- Complies with all hospital safety, infection control, and emergency response policies.
- Attends and completes all required education and training, including Safe Crisis Management (SCM) certification.
- Performs other related duties as assigned.
