What You'll Do:
- Conduct comprehensive Start of Care assessments and complete OASIS documentation timely
- Draft care plans, reconcile patient medications, and ensure care orders are accurate and complete
- Prepare accurate, orderly, and timely documentation
- Coordinate care with patients and provide proper reports to the Case Manager
- Provide health care instructions to patients and families as appropriate per assessment and plan
About You
- An active RN license in the practicing state or eligibility as a New Grad to take the NCLEX exam.
