Nurse Practitioner – Indiana, PA

We are seeking a Nurse Practitioner to join our medical team in the Indiana, PA region. The NP will primarily provide office‑based primary care and support long‑term care facilities. The position is a full‑time 4‑5 day per week schedule with potential travel across the region (~30 miles apart). Compensation up to $125k plus production bonuses and comprehensive benefits.

• Office‑based primary care
• Perform new admissions, consultations, and follow‑up care for long‑term care residents
• Round on patients, track progress, remove obstacles, and improve quality of life
• Engage with clinical staff regularly and maintain collegial relationships with facility physicians, caregivers, administrators, and directors
• Communicate clearly with patients and families to promote understanding of treatment options and procedures; set expectations with facility, patients, and families
• Attend facility team meetings as needed
• Follow documentation, billing, and coding protocols
• Meet regularly with facility leadership to understand needs and build relationships; report back to Director on facility happenings and updates
• Collaborate actively with treating physicians, providers, and treating groups (referrals, wound care, orthopedists, etc.)

• Graduation from an accredited Nurse Practitioner Program
• Active/Current NP license in PA
• Potential to travel between multiple long‑term care facilities (if desired)

• Full time (4‑ or 5‑day per week)

• Up to $125k to start plus production bonuses
