Licensed Practical Nurse (LPN) – Transitional Care Unit – Seneca, PA
Location:
UPMC Northwest Hospital, Seneca, PA – Transitional Care Unit. Status:
Casual (PRN). Shift: Flexible.
- The LPN is licensed to function as a member of the health‑care team by exercising sound nursing judgment based on preparation, knowledge, experience in nursing and competency. The LPN participates in the planning, implementation and evaluation of nursing care using focused assessment in settings where nursing takes place.
- The LPN shall communicate with a licensed professional nurse and the patient's health‑care team members to seek guidance when the patient's care needs exceed the licensed practical nursing scope of practice, when the patient's care needs surpass the LPN's knowledge, skill or ability, or when the patient's condition deteriorates or changes significantly.
- The LPN shall obtain instruction and supervision if implementing new or unfamiliar nursing practices or procedures. The LPN shall follow written, established policies and procedures of UPMC, UPMC LPN Practice Guidelines, and the State Practice Act, including IVs within the framework of Relationship Based Care.
- The LPN formulates daily goals and a plan of care for patients that involves the patient as a partner and considers the individual's needs holistically.
- The LPN communicates the patient’s condition to other care providers and ensures comprehensive documentation that promotes communication among caregivers.
- The LPN demonstrates caring and compassionate patient‑focused experience by building healing relationships with patients, families and colleagues.
- The LPN actively participates in unit activities, provides assistance to care team members, and delivers detailed teaching to patients and families to guide them through the episode of care and transition to another level of care or home.
- The LPN supports orientation and development of new LPNs and patient‑care support staff, seeks and applies continuous professional improvement, and participates in process improvement opportunities to enhance patient care.
- The LPN demonstrates accountability, cultural competence, and maintains a balance between work and personal life while communicating safety hazards and supporting continuous learning.
- * Performs in accordance with system‑wide competencies/behaviors. Performs other duties as assigned.*
- Graduated from a Licensed Practical Nursing Program approved by the State Board of Nursing.
- Minimum 2 years of experience in a medical/surgical/tele or emergency department setting.
- Successfully completed an IV therapy curriculum within 4 months of hire.
- Committed to obtain additional training or UPMC‑approved certification within the specified time frames.
- Demonstrated knowledge of patient development over the lifespan and independent assessment and intervention skills.
- Ability to identify and implement process improvement opportunities and collaborate effectively with the health‑care team.
- Physical stamina for frequent walking, standing, lifting and positioning of patients; computer skills for the electronic health record.
- Valid LPN licensure in the state of practice or a state covered by the Nursing Licensure Compact.
- Basic Life Support (BLS) or Cardiopulmonary Resuscitation (CPR) certification.
- Act 34 and OAPSA certificates (as applicable).
