Oversees all aspects of resident care in Personal Care. The resident care coordinator will be responsible for the overall process of resident assessments and support planning for the neighborhood. The Resident Care Coordinator must demonstrate an awareness that the needs of the residents and family members are first and foremost.
Specific Duties and Responsibilities- Assist the Director of Personal Care with pre‑admission assessment, resident assessment support plan and all required documents in accordance with current regulations.
- Participate in and/or lead support plan meetings on the unit and ensure staff who work with the resident attend.
- Make changes to and update the resident support plan based on physician orders, observations, patient response to treatment and data collected from all available sources.
- Communicate with physicians, including primary care physicians and specialists, as well as families and other responsible persons as appropriate.
- Monitor the administration of medications to residents according to facility policy.
- Participate in interviews and hiring of professional and non‑licensed staff, and assist with orientation and training.
- Supervise nurses and certified nurse aides, providing leadership, training, and coaching.
- Evaluate staff performance using accepted tools.
- Provide counseling staff on disciplinary issues.
- Investigate suspected abuse, neglect, or other resident care issues in a timely manner and report to administration.
- Oversee submission and follow‑up on all reportable incidents; monitor reports for trends and recommend procedural changes.
- Make recommendations to the Director of Personal Care during the budgetary process impacting financial health of the organization.
- Completion of Registered Nurse Program
- Nursing Degree, RN
- Three years experience in a nursing environment including care plan and supervisory experience.
- Five years experience in a geriatric setting with assessment responsibilities.
- Licensed as a Registered Nurse by the state of Pennsylvania.
