Base pay range: $41.00/hr - $41.00/hr

Pay: $41 per visit. Benefits include mileage and cell phone reimbursement, and 401(k) with match.

Director of Nursing.

At Home Quality Care was founded in 1983 in a rural area 50 miles west of Chicago. We are a leading provider of comprehensive home health and non‑medical home care services across Indiana, Illinois, and Texas. We deliver care with seven branches throughout the region and continue to grow.

• Provides direct patient care as defined in the state nurse practice act.
• Implements plan of care initiated by the registered nurse.
• Provides accurate and timely documentation consistent with the plan of care.
• Participates in the coordination of home health services, appropriately reporting the identified needs of other disciplines to the registered nurse and/or clinical supervisor.

• Home health experience preferred.
• Currently licensed as a Licensed Practical Nurse by the state.
• Possess and maintain current CPR certification.
• Licensed driver with current automobile insurance.
• Demonstrates good verbal and written communication skills.
• At least 18 years of age.
