- Active, unencumbered license (LPN/LVN) in the state of practice
- Compact licenses transferred within 90 days of state of residence
- Current CPR certification (hands‑on component). Aveanna can assist with obtaining this after hire, if necessary.
- TB skin test current within the last 12 months
- Six months prior hands‑on nursing experience preferred but not required
- Reliable transportation

Position OverviewThe Licensed Practical Nurse (LPN/LVN) provides and documents skilled nursing care, under the supervision of a Registered Nurse, following the care plan and physician orders while maintaining confidentiality and professional boundaries.
Essential Job Functions- Deliver and coordinate quality patient care in compliance with physician orders.
- Continuously observe and assess patient condition and care needs, reporting changes to the supervisor and/or physician as appropriate.
- Document all activities, assessments, nursing actions, responses, and coordination of care promptly.
- Participate, implement, and update the nursing care plan.
- Take appropriate nursing action based on assessment and achieve expected outcomes.
- Recognize changes in patient needs and respond promptly to prevent or reduce risk.
- Accept responsibility for personal and professional accountability, complying with Aveanna policies, state and federal regulations, accrediting bodies, and the Nurse Practice Act.
- Provide care using infection control measures that protect staff and patient per OSHA standards.
- Educate patient and family on disease process, self‑care techniques, and prevention strategies.
- Maintain knowledge of competencies related to the nursing profession through educational programs, CEUs, internal learning management, and skill evaluations.
- Graduate of an accredited school of nursing.
- Current, unrestricted state license as a Licensed Nurse in the state of practice.
- Current CPR certification.
- Demonstrated proficiency in clinical assessments, documentation, and compliance with nursing care policies and procedures.
- South Carolina – One (1) year of pediatrics experience.
- California – One (1) year of experience working under current nursing license.
- Louisiana – One (1) year of experience as a licensed nurse.
- Continuing education as required by state.
- Six (6) months of recent experience as a Licensed Nurse in a clinical care setting.
- Home health experience.
- Attention to detail.
- Time management.
- Effective problem‑solving and conflict resolution.
- Good organization and communication skills.
- Speak, write, read, and understand English.
- Travel ability.
- Lift 50 pounds.
- Reposition patients and move equipment without assistance.
- Prolonged walking, standing, bending, kneeling, reaching, twisting.
- Sit and climb stairs.
- Visual and hearing acuity.
- Strong sense of smell and touch.
- Respond physically and mentally to emergency situations in the home or during transport.
- Function in various environments that may involve exposure to allergens and other conditions.
- Possible exposure to blood, bodily fluids, and infectious diseases.
