Position: CHILD AND ADOLESCENT CASE MANAGER
Location: Bryn Mawr

Child & Adolescent Case Manager

Full-Time | Community-Based

The Child & Adolescent Case Manager supports children, adolescents, and families by connecting them to behavioral health, medical, educational, and community resources. This role uses a person-first, recovery-oriented approach to help youth achieve individualized goals.

Key Responsibilities
  • Provide case management services in homes, schools, and the community
  • Link families to mental health, medical, social, and educational supports
  • Develop and monitor recovery-oriented treatment plans
  • Coordinate care with schools, providers, and family members
  • Attend team meetings, school meetings, and clinical consultations
  • Maintain required productivity and complete documentation in EHR
Qualifications
  • Bachelor’s degree in sociology, social welfare, psychology, gerontology, anthropology, or related social sciences, criminal justice, theology, nursing, counseling, or education;
  • OR Be a registered nurse
  • OR Have a high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology, or other social science and two years in direct contact with mental health consumers.
  • Experience working with children/adolescents preferred
  • Knowledge of recovery, resiliency, and trauma-informed care
  • Valid driver’s license and access to a vehicle
  • Case Management experience preferred
