• Implement therapeutic exercises and training programs to enhance mobility and functionality.
• Educate patients and their families on injury prevention, wellness strategies, and recovery processes.
• Monitor and document patient progress toward rehabilitation goals, adjusting treatment plans as necessary.
• Collaborate with a multidisciplinary team of healthcare professionals to ensure comprehensive patient care.
• Maintain detailed patient records and documentation in accordance with regulatory standards.

• Master’s degree in Physical Therapy.
• Minimum of 1 year of experience as a physical therapist in a home health setting.
• Valid state licensure to practice as a physical therapist, adhering to professional standards.
• Current CPR certification required upon employment.
• Valid driver's license with the ability to travel to various client locations.
