BAYADA Home Health Care is currently seeking an experienced Occupational Therapist, OT for a full time home health opportunity. This office works with adult and geriatric patients on a per visit basis in senior living communities throughout Middlesex/Somerset County
.

As a home care Occupational Therapist, you will be an integral member of a multi‑disciplinary health care team that provides rehabilitative care and skilled nursing to clients, allowing them the opportunity to remain at home.

• Perform outpatient therapy evaluations to develop a plan of care for patients in designated senior living communities or group homes for adults with intellectual disabilities.
• Operate at the top of licensure, utilizing current research‑based approaches to maximize functional independence for clients.
• Work closely with home health therapy partners to transition care from home health to outpatient services for client‑centered goals.
• Educate clients, families and caregivers about care plans to maximize functional independence.
• Perform evaluations, treatments and proper discharge planning, documenting on the EMR platform.

• Current NJ Occupational Therapist license.
• Prior home care experience preferred, but not required.
• Minimum of one year of recent work experience as an Occupational Therapist.
• Graduation from an occupational therapy program accredited jointly by the Committee on Allied Health Education and Accreditation of the AMA and the AOTA.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational and problem‑solving skills.
• Solid computer skills; experience with EMR preferred.

Base Pay, depending upon qualifications and subject to negotiation: $60.00 - $68.00 per point
