We are seeking a licensed and motivated Occupational Therapist to join our home care team. As an OT, you will play a critical role in helping patients regain independence and improve their quality of life in the comfort of their own homes.
Role OverviewThe Occupational Therapist will evaluate patients in their homes, develop personalized care plans, and implement therapeutic strategies to promote safety and functionality. Working alongside a multidisciplinary team, you’ll help clients adapt to daily challenges and achieve their highest level of independence.
Responsibilities- Conduct in‑home assessments of patients' physical, cognitive, and functional abilities
- Develop individualized treatment plans focused on improving daily living and motor skills
- Provide direct therapeutic interventions and adaptive equipment training
- Educate patients and caregivers on strategies to promote safety and independence
- Recommend home modifications and equipment as needed
- Collaborate with nurses, physical therapists, social workers, and aides for coordinated care
- Participate in care planning and team meetings as required
- Current New York State Occupational Therapist license
- Graduate of an accredited Occupational Therapy program
- Minimum of 1 year experience in home health, rehab, or clinical setting preferred
- Strong assessment, clinical, and communication skills
- Ability to work independently and manage time effectively
- Reliable transportation and willingness to travel to patient homes
- CPR certification and up‑to‑date health requirements
