BRIEF

JOB DESCRIPTION

The Occupational Therapist is responsible for providing occupational therapy services and for emphasizing health education/experience. The therapist is responsible for traveling to a patient’s home to administer the services dictated by their physician while helping patients maintain their independence and quality of life. This role provides a lot of flexibility for our field staff!

Essential Job Functions/Responsibilities
  • Participate in the ongoing evaluation of patient’s functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self‑care ability, work capacity, etc.) as delegated by the registered occupational therapist. Participate in the ongoing evaluation of the home environment for hazards or barriers to more independent living as delegated by the patient’s plan of care.
  • Participate in teaching new skills or retraining patients in once familiar daily activities that have been lost due to illness or injury, in accordance with organization policy.
  • Maintain appropriate clinical records, clinical notes, and report to the branch’s Director of Nursing any changes in the patient’s condition.
  • Submit clinical documentation in accordance with company policy.
  • Follow treatment program and goals for improved patient function as established by the Plan of Care. Document patient’s response to treatment plan and progress toward established goals.
  • Maintain contact/communication with other personnel involved in the patient’s care to promote coordinated, efficient care. Document such communication in accordance with company policy.
  • Attend and contribute to in services, case conferences, and other meetings as required by company policy to ensure coordinated and comprehensive plans of care for the patients of the company.
  • Identify patient and family/caregiver needs for other home care services. Consult with other clinical disciplines and assist with necessary referrals, as appropriate.
  • Participate in instructing patient’s family/caregiver and other health care personnel in patient’s treatment regime as delegated by the registered occupational therapist.
POSITION QUALIFICATIONS
  • Graduate of an occupational therapy curriculum accredited by the American Occupational Therapy Association.
  • Licensed in accordance with state licensure laws.
  • A minimum of one (1) year of clinical experience as a certified occupational therapist.
  • Demonstrates good verbal and written communication, and organization skills.
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working order.
