Bradford Hills Nursing & Rehab Center is seeking a dedicated Full-Time Physical Therapist (PT) to join our outstanding in-house rehabilitation team. You will play a key role in enhancing residents’ mobility, function, and quality of life. You’ll evaluate, develop, and implement treatment plans tailored to each individual’s goals—helping them regain independence and strength.
Key Responsibilities:- Perform initial and ongoing assessments of residents' physical conditions and functional abilities
- Develop and carry out individualized treatment plans to reduce pain, restore mobility, and prevent further injury
- Document all patient interactions, assessments, and progress in the Net Health EMR system (preferred)
- Collaborate with caregivers, nursing staff, and interdisciplinary team members
- Provide patient and caregiver education on treatment strategies and home programs
- Monitor and adjust treatment based on patient response and progress
- Ensure compliance with all federal, state, and facility policies and procedures
- Current PA license as a Physical Therapist in good standing (required)
- Experience in SNF/long-term care settings preferred
- Knowledge of current therapy best practices and protocols
- Experience with Net Health EMR preferred
- Ability to work with patients of varying conditions and personalities
- Strong communication, organizational, and interpersonal skills
