Pleasant Acres Rehabilitation and Nursing Center is seeking a compassionate and skilled Physical Therapist (PT) to join our in-house rehab team! This is an exciting opportunity to work in a collaborative environment where patient care comes first and professional growth is encouraged.

Key Responsibilities:
- Evaluate and develop individualized treatment plans for residents with temporary or chronic physical conditions.
- Deliver skilled physical therapy services to promote healing, reduce pain, and improve mobility and function.
- Monitor and document patient progress, adjusting treatment plans as needed.
- Educate patients, families, and caregivers on home exercise programs, injury prevention, and rehabilitation techniques.
- Collaborate with nursing, therapy, and medical teams to ensure coordinated care.
- Complete documentation accurately and in compliance with company policies and regulatory guidelines.
Duties Include:
- Performing evaluations and interpreting test results.
- Implementing a variety of therapeutic treatment techniques.
- Advising patients on proper movement, ergonomics, and lifestyle habits.
- Developing personalized, goal‑driven rehabilitation programs.
- Ensuring adherence to all applicable rules, procedures, and state/federal regulations.
- Licensed Physical Therapist in the state of Pennsylvania (or eligible)
- Experience in Skilled Nursing Facility (SNF) or long‑term care preferred, but new grads welcome!
- Strong clinical skills and a passion for delivering high‑quality, ethical care
- Excellent communication and interpersonal skills
- Organized, self‑motivated, and a true team player
