Complete Description:
The Registered Nurse applies knowledge of nursing theory, technical, and communication skills in delivering high quality compassionate care to patients and families. The RN assesses patient need, develops a plan of care in collaboration with the interdisciplinary team, provides direct care, evaluates patient progress, and coordinates the care delivery throughout the patient's episode. The Med/Surg RN develops specialized skill in the area of cardiac care including telemetry monitoring.
The RN is accountable for patient family education to assist the patient and family in understanding self care beyond hospitalization, including lifestyle changes to support health and healing. He/She promotes comfort and provides emotional support in a healing environment.
Applies nursing knowledge and skill in the delivery of care Applies nursing knowledge and skill in delivery of care by assessing patient /family needs through active listening, social perceptiveness (being aware of others' reactions and understanding why they react as they do), and analyzing data/information obtained through direct observation and assessment, technology, or health team communication. Creates a developmentally and age appropriate, culturally relevant, individualized plan of care to address the nature and scope of the health problems identified.
In collaboration with the patient, family, physician, and health care team members, sets goals and monitors the patient's progress. The RN adjusts the plan based on patient response or change in condition/reprioritization of needs.
Promotes, advocates for and protects the health, safety and rights of the patient. Systematically evaluates quality and effectiveness of Nursing Practice, uses the results of Quality Improvement activities to initiate changes in individual nursing practice and to influence care delivery. Engages in self-evaluation of practice, identifying strengths and areas in which professional development would be beneficial and takes accountability to increase knowledge and enhance skill.
Uses logic and reasoning to identify the strengths and weaknesses of alternative solutions, conclusions, or approaches to problems. Demonstrates skill in application of nursing interventions including medication administration.
Motivates the patient/family by teaching them what is needed to continue care and healing esses the patient/family learning styles and structures the sessions to maximize understanding. Seeks "teachable” moments during the care delivery process to emphasize pertinent information. Asks questions to evaluate retention of key points in the self care process. Provides tools and information to assist the patient family after discharge.
Documents the education provided and the patient/family response to the education.
Encourages and builds mutual trust, respect, and cooperation among team members to promote members of the team working together to accomplish the best possible patient outcome. Teaches others as appropriate and delegates tasks and interventions consistent with the individual's education, training, and scope of practice. Complies with infection prevention strategies and maintains competencies required to care for the specific population in the area assigned.
Reports potential safety risks and inappropriate/questionable practices. Promotes leadership awareness and fosters positive change. Conducts rounds per protocol and uses tools and equipment needed to keep the patient/family informed, engaged, and safe.
Nursing practice reflects the importance of making a meaningful connection with patient/family.
Seeks to develop a helping, trusting, and caring relationship by being present in the moment of interaction. Listens, communicates, engages, and anticipates the patient/family needs. Strives to understand what is most important to the patient/family, acknowledging that the patient and family are the central focus of all care and services.
